The Patient tab holds everything you know about the patient that is not part of the call itself. Their identity, contact info, family, history, medications, allergies, and immunizations all live here.

The sections on this tab
- Patient Info — name, date of birth, address, phone, race, ethnicity, gender.
- Parent / Guardian / NOK — used for minors and unconscious patients.
- Drivers License — license number and state, when relevant.
- Patient Practitioners — primary care doctor and specialists.
- Immunizations — vaccinations the patient has had.
- PMH — past medical history, allergies, and advance directives.
- Medications and Supplements — what the patient is currently taking.
You do not need to fill in every section for every patient. Fill in what is relevant to the call.
Tip: If a patient is unconscious and you do not know their name, at minimum fill in the date of birth (even approximated) and any ID you can find. A good Patient Info section helps the hospital pull their chart faster, and your billing team use patient-matching services to fill in the rest later.
