The PMH section is where you record everything about the patient that predates this call: their conditions, their allergies, and any advance directives like a DNR.

Past medical history

Pick from the standardized list — diabetes, hypertension, COPD, CHF, atrial fibrillation, prior stroke, prior MI, and so on. You can pick more than one. There is also a free-text field for anything not in the list.

Allergies

  • Pick the kind of allergy: drug, environmental, or food.
  • Enter what the patient is allergic to.
  • Enter the reaction if known.

Advance directives

Mark whether the patient has a DNR, POLST, MOLST, living will, or healthcare proxy on scene. If you have a DNR in your hands, take a photo of it and attach it to the Documents tab.

An example

You respond to a 78-year-old with shortness of breath. The patient tells you he has CHF, COPD, and hypertension. He has a penicillin allergy that gives him a rash. His daughter is on scene with a POLST form that says comfort measures only. In the PMH section you check CHF, COPD, and hypertension. You add a drug allergy for penicillin with "rash" as the reaction. You mark POLST present in the advance directives. You take a photo of the POLST and attach it to the Documents tab.
