The Parent / Guardian / Next of Kin section is where you record someone other than the patient who can be contacted about the call. This is most often used for minors (parents) and unresponsive adults (next of kin).

What to enter

  • Their relationship to the patient.
  • Their name.
  • Their phone number.
  • Their address, if you have it.

An example

You respond to a 6-year-old having a seizure at school. The school nurse hands you the parent's phone number on a sticky note. You add the parent as Mother, type her name and phone number, and call her on the way to the hospital. The phone number stays on the PCR for the receiving facility to use.
