Every PCR has its own audit log. The log shows who has opened the report, who has changed any field, and what the change was.

Open the audit log
- Open All PCRs.
- Find the PCR you want to audit.
- Click the Actions menu on the row and pick Audit Log.
What you see
The audit log opens as a modal with one row per action. Each row shows the Timestamp, User, Action (viewed, submitted, PDF generated, fax sent, or field change), Section, Field, Old Value, New Value, and IP Address. Field changes fill in the old and new values so you can see exactly what was edited.
Who can see it
The Audit Log action only appears on All PCRs and is limited to administrators.
When to use it
- HIPAA compliance — proving who has and has not seen a patient record.
- Investigating a discrepancy — when two crew members remember a call differently.
- Responding to a subpoena — showing the chain of edits to a PCR.
- Quality reviews — showing exactly when a quality reviewer requested changes and when the original provider responded.

An example
A PCR comes back from the billing team with a question about why the destination was changed. You open the audit log and see that you originally entered the closest hospital, then changed it to the trauma center after consulting medical control. The audit log shows both versions and the timestamp of the change. You reply to the billing team with the explanation, citing the audit log entry.
