The Destination / Transfer section is where you record the facility you took the patient to and why you picked it.
What to fill in
- Destination — the facility, picked from your agency's facilities list.
- Type of destination — hospital, dialysis, urgent care, residence, morgue, etc.
- Reason for choosing destination — closest, patient choice, specialty (cardiac, stroke, trauma), insurance, family request.
- Hospital capability — STEMI center, stroke center, trauma level, burn center, pediatric capable.
Why "reason" matters
If you bypassed the closest hospital to take a STEMI to a cath-capable center, the reason field is what shows you made the right call. Without it, the bypass looks like a longer transport time on a report. With it, the report explains why.
An example
You have a patient with a confirmed STEMI on 12-lead. The closest hospital is a community ED 4 miles away, no cath lab. The regional STEMI center is 18 miles away. You bypass the closest hospital and transport to the STEMI center. In the Destination section you pick the STEMI center, mark the type as Hospital, mark the reason as Specialty - Cardiac, and mark the hospital capability as STEMI Center. The longer transport time is fully justified.
