This report lists every procedure attempt that ended in failure (eProcedures.06 = "No"), grouped by provider and procedure type. Failed intubations, missed IVs, unsuccessful IO placements, that kind of thing.

How to use it

  1. Open Operations → Provider Quality → Failed Procedure Log.
  2. Pick a window — at least 3 months for meaningful counts.
  3. The grid shows provider, procedure name, and failure count, sorted by count.

What to do with this

Failed procedures are normal — even highly skilled providers miss IVs and have difficult intubations. The report is a training-opportunity surfacing tool, not a disciplinary one:

  • A provider with repeated failures of the same procedure may benefit from refresher training.
  • A specific procedure that fails frequently across many providers may indicate an equipment problem (e.g. a bad batch of laryngoscope blades).
  • Pair the failure count with the success count for context — five failed IVs is meaningful in different ways for a provider with twenty IV attempts vs one with two hundred.
