The seven statuses

Every incident report has one of these statuses. Understanding them helps you know what needs to happen next.

Draft

The report is still being written. No one has reviewed it yet. You can edit it freely. This is the starting status for every new report.

Pending review

The author has finished the report and sent it for review, but no reviewer has started looking at it yet. It is waiting in the queue.

In review

A reviewer is currently examining the report. They will either approve it or reject it with notes.

Approved

The reviewer has determined the report is accurate and complete. It is now ready to be submitted to the national system.

Rejected

The reviewer found issues that need to be fixed. The report goes back to the author. Check the QA and Reviews tab to see what the reviewer said needs to change. Once you fix the issues, you can resubmit for review.

Submitted

The report has been sent to the national incident reporting system. The data is now part of the national record.

Failed

The system tried to submit the report, but something went wrong. Check the Submission tab on the detail page for error details. Common reasons include missing required fields that the local validation did not catch, or a temporary connection issue with the national system. Fix the issue and try submitting again.

How reports move between statuses

Reports follow a general path:

  1. Draft -- You write the report.
  2. Pending Review -- You click Submit for Review.
  3. In Review -- A reviewer picks it up.
  4. Approved or Rejected -- The reviewer makes a decision.
  5. If rejected, it goes back to Draft or Pending Review after you fix it.
  6. If approved, it moves to Submitted once it is sent.
  7. If the submission fails, it moves to Failed. You can fix it and try again.
Tip: Check the dashboard regularly for any reports in the Pending Review, Approved, or Failed statuses. These are the ones that need attention to keep your reporting pipeline moving.
