Go Live checklist for Facility Transport Hub

A short checklist to walk through the first time you sign in to Facility Transport Hub — verify your facility info, set up alerts, and invite your team.

Updated Jun 2, 2026

This is the short setup checklist to run through the first time you sign in to Facility Transport Hub. Once these few items are done, your facility is ready to file transports and respond to your dispatcher.

The structured checklist below is the working version. Open each item and check it off as you go.

Some of the steps require facility-admin access — those are the steps that affect everyone at your facility. If you do not have facility-admin access, ask the person who invited you.

Once the checklist is done, the rest of the wiki is reference material. The articles under Requesting a transport and Managing your requests cover the daily work.

Go Live checklist

  1. 1

Sign in for the first time

    • Open sso.unified-apps.com and click the Facility Transport Hub tile.
    • If this is your first time, your transport agency invited you by email — open that email and click the one-click setup link to set your password before you sign in.
  2. 2

Confirm your facility name and address (facility-admin only)

    • Open Settings, then Facility, from the user menu.
    • Verify the facility name, phone, and address are correct. These details are pre-filled on every transport request you submit, so the dispatcher sees exactly what is on this page.
    • This step requires facility-admin access. If you do not have it, ask the person who invited you to confirm the facility info.
  3. 3

    Verify your phone number

    • Open Profile from the user menu. If your phone number is not verified, you will see a verify-now banner.
    • Click the banner, request a code, and enter the six-digit code that arrives by text. Once verified, text-message notifications can reach you.
  4. 4

    Set your notification preferences

    • On the Profile page, scroll to Notification preferences.
    • For each event — counter-offers, confirmations, declines, cancellations, completions — toggle email and text-message on or off, independently.
    • A common starting point is text-message for counter-offers and confirmations, email for everything else. You can change these any time.
  5. 5

Invite other coordinators at your facility (facility-admin only)

    • If other staff at your facility will also file transports, open Settings, then Users.
    • Click Invite, enter their name and email, and choose whether they should also be a facility-admin.
    • They receive a one-click invitation email and can sign in once they set their password.
  6. 6

File a test transport request (optional)

    • Click New transport request and walk through the form to make sure everything looks right.
    • You can cancel the request after submitting if you do not need an actual pickup. This is the easiest way to confirm the form behaves the way you expect before a real patient is involved.
