Every transport request includes patient information. You only need three of the five identifying fields, but more is better — it makes it easier for the dispatcher to match the patient with their existing records.

The five fields

  1. First name
  2. Last name
  3. Date of birth
  4. Last 4 of SSN
  5. Home address line 1

Three of these are the minimum required for the request to go through. A common combination is first name, last name, and date of birth. If you have access to the patient's social security number or home address, include those too.

Looking up an existing patient

If you have transported this patient before, click Search records. The system looks up the patient in your transport agency's records and pre-fills what it has on file, so you do not have to type the same information twice.

Multiple patients

Each transport request is for one patient. If you need transport for multiple patients, file a separate request for each, since each transport has its own time, route, and crew.
