The Transport Information section is where you record the details of how you got the patient to the destination.
What to fill in
- Transport mode from scene — emergent (lights and sirens), non-emergent, or no transport.
- Position of the patient during transport — supine, semi-Fowler's, lateral, prone.
- Number of patients transported in this unit.
- Disposition — for the call as a whole. The most common is "Transport by EMS, no additional transport." Other options include refusal, no patient found, treat and release, deceased, and transferred to another EMS unit.
An example
You transport the chest pain patient emergently to the STEMI center, in a semi-Fowler's position, one patient. In the Transport Information section you pick Emergent, Semi-Fowler's, 1 patient, disposition Transport by EMS.
