When you add an item to a template, you pick a field type and then configure it. Each field type has its own settings.

Settings available on every field type

  • Placeholder — gray text inside the field that disappears when the person starts typing. Use this to show the expected format, like "e.g., 12345" or "Describe the damage."
  • Helper text — a short note that appears below the field label to guide the person filling it out. For example, "Check the expiration date on the label" or "Enter the four-digit lot number."
  • Required response — tick this box to require an answer before the session can be submitted. Use this for items that should never be skipped.

Settings by field type

Pass / Fail

You can change the Pass label and Fail label to anything you want. For example, "Good" and "Needs Repair" or "Present" and "Missing." You can also tick Require explanation for either outcome, which makes the person add a short note when they pick that option.

True / False

You can change the True label and False label. The defaults are "Yes" and "No." Like Pass/Fail, you can require an explanation on either answer.

Short answer

Accepts up to 255 characters. Use the placeholder to show the expected format.

Paragraph

Accepts up to 2,000 characters. Good for open-ended notes.

Count

You can set a minimum and maximum value. For example, an oxygen tank PSI reading might have a minimum of 0 and a maximum of 4000.

Fuel gauge

The default stops are E, 1/4, 1/2, 3/4, and F. You can edit, add, or remove stops as long as at least two remain.

Multiple choice

You add the list of options. You also choose whether the person can pick only one option or more than one.

File upload

You can set the maximum number of files and the allowed file types (for example, image/* and application/pdf).

Signature

No extra settings. The signature pad appears as-is.

An example

You are building a template for a narcotics count. One item is "Number of fentanyl vials." You pick the Count field type, set the minimum to 0 and the maximum to 20, tick Required response, and add helper text that says "Count all vials in the lockbox, including partial vials." Another item is "Lockbox seal intact." You pick Pass / Fail, change the labels to "Intact" and "Broken," tick Required response, and tick Require explanation on the Broken option so crews always explain what they found.
