Any authenticated user can file an accident report for any vehicle in the fleet. File a report for any contact with another vehicle, object, or person — even a parking-lot scrape. Documenting promptly protects you and your agency.
How to submit a report
- On the Fleet Management dashboard, click Report Accident in the Accident Reports panel.
- The Submit Accident Report modal opens. It is organized into three sections.
- Fill in every required field.
- Click Submit.
Incident Details
- Vehicle (required) — the vehicle involved.
- Accident Time (required) — when it happened.
- Street Address or Intersection (required) — where it happened. Be specific.
- Weather — Clear, Rain, Snow, Ice, Fog, High Winds, or Other.
- Road Conditions — Dry, Wet, Snow / Slush, Ice, Loose Gravel, Debris Present, or Other.
- Vehicle Speed (MPH) — the vehicle's speed at impact.
- Accident Narrative (required) — a plain-language description of what happened.
- Vehicle Damage Summary — what damage you can see.
People & Response
- Any Injuries? — toggle on if anyone was hurt.
- Injuries / Medical Treatment — only shown when Any Injuries is on. Describe who was hurt and what treatment they got.
- Police Report # — the number if police filed a report.
- Responding Agency — which agency responded.
- Witness Contact Information — names and phone numbers.
Vehicle Media
You can attach up to 20 photos and 6 videos.
- Click Add Photo for each photo. For each one, pick the Vehicle Area (Front, Rear, Driver Side, Passenger Side, Interior, Roof, Undercarriage, Engine Bay, or Other), add a Photo Label and Photo Notes, then upload the image.
- Click Add Video for each video clip. Add a Clip Description, then upload the video file (MP4, QuickTime, or AVI).
What happens next
The report goes to your agency's admin team on the Accident Management page. They coordinate repairs, file insurance, and track the case through to close.
From the Unified dashboard
You can start an accident report from the Unified Solutions dashboard as well. Click the + button, pick New Accident Report, and you are dropped into the Fleet Management report form with the modal open.
Tip: Photograph the damage before you leave the scene. Take photos of the whole vehicle, then close-ups of any damaged area. Videos are useful for dash-cam footage or drive-cam recordings.
