Each item in a checksheet has a field type that controls what kind of answer it expects. Here is every type you may see.

Pass / Fail

Two buttons, usually labeled Pass and Fail. Your administrator can change these labels to anything that fits, like "Good / Bad" or "OK / Needs Repair." Use this for any item where you are checking whether something meets a standard.

True / False

Two buttons labeled Yes and No by default. Your administrator can change these labels to any two options that fit. Use this for yes-or-no questions, like "Fire extinguisher is mounted in bracket" or "AED pads are in date."

Short answer

A single-line text box for brief answers, up to 255 characters. Use this for things like equipment serial numbers, odometer readings, or a quick note.

Paragraph

A larger text box for longer answers, up to 2,000 characters. Use this when you need to describe a problem, write a note about a repair, or explain a situation in detail.

Count

A number field. Your administrator can set a minimum and maximum value. Use this for counting supplies, like "Number of 4x4 gauze packs" or "O2 cylinder PSI reading."

Fuel gauge

A set of buttons that represent fuel levels: E, 1/4, 1/2, 3/4, and F. Use this for recording the fuel level in a truck, generator, or any equipment with a tank.

Multiple choice

A list of options to pick from. Depending on how the template is set up, you may be able to pick only one option or more than one. Use this for things like "Condition of tires" with options like "Good," "Fair," "Needs replacement."

File upload

A button that lets you attach a photo, document, or other file. See the uploading files article for details on allowed types and limits.

Signature

A signature pad where you sign with your finger or stylus. See the capturing a signature article for details.

An example

Imagine a "Narcotics Count" checklist. It might use a count field for the number of vials, a pass/fail field for whether the seal is intact, a short answer field for the lot number, a signature field for the person counting, and a signature field for the witness. Each field type fits the kind of information the item is asking for.
